The ilioinguinal nerve was identified lying on the cremaster.

A nick was made on either side of the nerve and the nerve was gently elevated with a right angle clamp and freed from the cremaster.

The mobilized nerve was most easily protected outside the medial flap of external oblique. Either flap may be used depending on the disposition of the nerve.
