The cord was elevated at the pubic tubercle by grasping the cremaster. A Pean clamp was used to follow the lateral crus under the cord and free it from the pubic tubercle. A similar spreading action above the cord completes the tunnel without the need for traumatic finger dissection.

A Penrose drain was passed under the cord for retraction.

A small opening was made sharply in the cremaster and the remainder opened using cautery.
