The hernia sac was identified on the anteromedial surface of the cord and sharply separated from the cord structures.

Dissection was continued with cautery.

The sac was mobilized as much as possible toward the deep inguinal ring. Palpation revealed firm stool indicative of sigmoid colon in the sac.
